Step 2. Contact the dental clinic directly to book an appointment

Step 3a. Before the appointment, complete the Application and Consent Form and the Oral Health Questionnaire

Step 3b. Complete registration in eHealth

* If you have not enrolled, adolescents choose to do so on the date receiving the subsidised services when accompanied by parents or guardians, with the assistance of the clinic staff.
